Visual Migraines and Vertigo caused by head position

I am a 60 y/o woman in general good health.  I am living in a small town in Costa Rica.  I have in the past been seen by an opthalmologist (in US) for what was diagnosed as Visual Migraines...where my eyes would seem to have a visual blank spot or lines of print would seem to move on a portion of a page of print.  I have only had pain and light sensitivity (regular migraine?) 1 - 2 times a year.  I had one of this type of migraine a couple of weeks ago.. but other wise no problems.I have not been on anytype of medication.  Normal BP,Glucose,Chemistry panel.

5 days ago I was reading in bed, turned my head to the left and felt a strange twinge..not painful.. but my head began spinning rapidly and I broke into a sweat.  When I could sit up, all the vertigo stopped, but would start again if I tried to lie down.  No pain, no symptoms of stroke.. some nausea. A local GP said he thought the vertigo was caused by stress, and my blood pressure had risen from normal to 190/100 when he examined me about 6 hours after the attack. He prescribed Tafil(xanax) for stress, Vontrol and Ginkgo Biloba extract, and Plendill 5 mg for BP.  I had minor attacks of vertigo each night when lying down but getting better.  This morning I turned my head to left while lying down and now I have vertigo standing or lying down.  Could this be another type of Migraine? Inner ear infection?  Pinched nerve in neck? I can not drive like this but could take taxi to  a larger city... Would an ENT specialist be where I should go?

unlikely to be migraine in my opinion.  much more likely to be an inner ear problem.  so yes, sounds like an ENT would be a good place to start.
