Asthenopia and Migraine

Dr Oyakawa,
Just want to follow up a question answered by you 2 days ago.
The cataract surgeon I met last week told me to test with the contact lens first before making the decision of the cataract surgery. He just mention there can be many other causes of headache like ocular migraine.

I am using a -3.25 glasses for my left eye which I can see 20/60. I was examined by a few optometrist, eye doctors in HK. The best I can see (almost guess) is 20/50 with -4.75. I tried to add a contact lens with -1.5 for 1/2 day but didn't help much to my eye strain and headache. I guess the major reason is with the cataract, my distant vision is blur with or without the contact lens.

Now, I am not sure with this test result, if there is any indication that the cataract surgery won't help.
Dr Oyakawa : You didn't mentioned 'Many patients' in a previous posting. Do you mean many patients like me have eye pain or asthenopia go for a cataract surgery and have improvement. Finally, anyway to test do I have Ocular migraine. My family doctor prescribed me a few Zomig and didn't work out.

Thanks a lot

Part of the problem may be due to the phoria.  See my response to your previous post today.  I do not believe you have a good refraction.  There is no correction for astigmatism.

Dr Oyakawa,
I asked the same question on the other post. Please ignore this one if you already answered.

The cataract surgeon I met last week just told me that I need more testing to ensure my problem is from the cataract before making the decision for a surgery.

I have astigmatism prescription in my glasses. Recently, I was tested by 2 optometrists/eye doctors with 2 eye exams each last for more than 1/2 hr. With the best correction of the lens including myopia and astigmatism, I can only see 20/50. But I can see a pin hole with 20/25. The eye doctor in the clinic then draw the conclusion that my current limit is 20/50 and if removing the cataract, my vision can become 20/25 or better. His explanation is that a pin-hole test is like a PAM test where the image will not be disturbed by the cataract.

Dr Oyakawa:
Do you mean I should I ask for another evaluation using corneal topography to ensure there is no uncorrected astigmatism ?
How likely is that the cataract limit my vision to 20/50 and there is no way to correct by lens ?

Thanks
