Crystalens and patient pupil size

hi ,

this question pertains tto what are the pupil size limitation while considering a crystalens implant - given that the crystalens HD has a 5 mm optic , what is the limit on how large the pupil size can be in order to be a good candidate for a crystalens implant . Can the small optic size cause severe  night vision issues to patients larger than this limit?

Hello Dr. Oyakawa,

I have been to three opthalmologists for opinions about my cataracts - two of them reccomended using the Crystalens - and  now looking at copies of the medical reports  from those visits  i see they measured my pupils to be 3 -5 mm in bright versus dim light. I explicitly asked one of them if my pupils were too large for the crystalens and he said that would not be a problem for pupils of my sizes,

The third opthalmologist i went to said my pupils were too large - and that they dilated to 7.5 mm in dim light -  i was surprised to hear this since i thought the earlier two surgeons had already looked into this.

I am a little confused , Could you suggest what i should do here? Should i seek a fourth opinion or should i go back to the first two surgeons and ask them again?

According to Dr. Holladay, an optics guru, the previous AT45 (4.5 mm optic) had an equivalent diameter of 6.0 mm at the standard position of IOL due to its more posterior location.  I would guess that the 5.0 mm Crystalens would have more the 6.0 mm at the standard IOL position.

Pupils over 6mm in darkened condition may have more problems.Larger pupils would have more visual symptoms that smaller pupils.  The multifocal IOLs would probably have more problems with large pupils than Crystalens.
