It is a grid pattern printed on paper that you look at one eye at a time to test your central vision which correlates to the macula anatomically.  The grid has a dot in the dead center and when you look at it - that spot will be fixated exactly on the center of your macula.  Any distortions, blurred area, bent or curved lines - will correlate to physical abnormalities of the macula.  It's sort of like a personal self-test for macular disease and is most useful for monitering the vision for any new changes.  If new changes are noted, the ophthalmologist should be notified right away.
