Slanted and tapered vision

Recently I started observing some distortion in my vision. I started noticing this when I got my new prescription glasses as part of routine eye examination. Prior to that I did not have this issue.I later on did re-exam with a different doctor and found that the prescription had left and right swapped. I now got it corrected and following is my latest Rx:
       Sphere Cylinder Axis
OD  -3.00    +.50       090
OS  -2.75    Sphere
Co-incidentally a month ago,I did hit my forehead (area between my eyebrows) hard against a table edge in my sleep. There was a small swelling and felt dizzy for a bit.

My problem is that any rectangular shaped objects such as computer monitors, browser windows would appear tapered from right to left such that the right hand side of the object looks bigger than left hand side.
i.e. the top horizontal line of the rectangular shaped objects seems to be slanting upwards from left to right resulting in a tapered effect. If I adjust my head/eye to see the top horizontal normal then the bottom horizontal line would seem to be slanting downwards from left to right. This kind of perception change would happen based on my orientation but majority of the times its the top horizontal line that would slant upwards.
I don't see the lines curving or wavy. It looks straight but just slants. I have attached an image(https://drive.google.com/file/d/0BzKaRtuocVolVmlVZ0pqenlCUnM/view?usp=sharing) to describe my issue. I perceive rectangular shapes as shown in the image.

I am not sure what is going on here since this issue is happening irrespective of whether I wear glasses or not.
I would be grateful if you could let me know what is happening with my vision and what could be the possible cause. Please provide me some directions in order for me to find solutions to my problem.

I have been getting quite a lot of follow up questions from other members regarding similar issues. Thought I would post an update on what's going on with me based on my situation. My issue was related to me hitting my forehead hard against a table.
I went to a lot of opticians and optometrist. Nothing solved. After seeing 4 different doctors I got referred to a Neuro Opthamalogist.
Luckily it was not related to optic nerve damage. It was a relief that I got checked. It was more of a trauma from head hitting hard and it created a temporary issue both eye related and psychological and that I had to retrain my eyes looking at shapes again and forcefully think that shapes are correct. So basically retraining and reinforcing. Right now the only part that is remaining is psychological issue which is possible due to sudden shock and your brain starts accepting the newly perceived shape and functions that way. This needs to be changed and that is why it’s important to retrain your brain and eyes. It is a bit time taking process to retrain . For me it took almost 2 years to retrain and come out of this issue.  It will happen eventually if you are determined.. As long as I don’t think about it the issue don’t exist. Yoga and meditation helps a lot as it helps with stress release.

That information is useful.  I think you would need to see an ophthalmologist that specializes in cornea and refractive surgery. Certain special tests would need to be done:  1. Ultra-careful refraction 2. corneal topography to rule out irregular astigmasim and diseases such as keratoconus  3. careful exam of lens to be sure no early cataract  4. repeat amsler grid  5. macular OCT to rule our macular disease.

Hi John - Thanks for taking the time for looking into this. Here is my response:

1) I notice this in both eyes with and without wearing glasses.
2) I looked at a square without wearing glasses. I cannot confidently say that its not happening. I do see something different. i.e. the effect is there but not very pronounced as in seeing rectangular shaped things.
3) I am seeing this effect with my old glasses as well. This is really surprising to me since I never noticed this before I started wearing the wrong prescription glasses.
4) I am 31 y.o and I would say I am pretty healthy individual with no major ailments in the recent times.
5) First visit - I think he is a doctor part of an optical shop
Second visit - He is an Eye MD ophthalmologist for sure
6) I read about Amsler grid test online and did the test for each eye individually with and without glasses.
I tested negative. I was able to see the grid just fine. I did not see any wavy or blurry lines or any missing spots.

Some questions: 1. Do you notice this in both eyes. 2. Look at a square without your glasses. Does it still appear that way. 3. Do you see this with your old glasses also? 4. what is your age and health status?  5. Where you checked by an optometrist or an Eye MD ophthalmologist.   6. Search and read about "Amsler Grid testing" Print out an amsler grid and test each eye separately with the grid and not what it looks like.

Let  me know your answers and I can give you a better idea of what the problem may be.   JCH MD
