Vitrectomy after Cataract IOL Replacement

Dear Doctor:

What happens if I go ahead and have the Vitrectomy after the IOL Replacement?  I have already done this in my right eye and with the lens in my left eye the floaters are even bigger?  Is the success rate for the Vitrectomy after the IOL good?  Also what happens down the road in my later years (I'm 56) if the Vitreous is removed and the surgeon put saline solution in my eye?  He said my body will replace this later, but what happens to my eyesight with the vitreous removed?

I'm having the Vitrectomy for opaque floaters that I cannot see through.  They are bigger now with the IOL implant.

This is a genralization but vitrectomies are normally not done for floaters. There are several people that have posted here that have had vitrectomies for floaters and have not been help, one was worse, all were unhappy.

I would recommend strongly you get an independent second opinion.

Dear Doctor Hagen:

I do feel my eye with the floaters removed gives me better vision when I'm in lights of any kind.  I had cataract surgery without having the floaters removed first in the other eye.  Now the lights nearly blind me as the cataract lens makes the floaters 2 times larger.  I have poor vision anyway and having the floaters removed in one eye helped me greatly and I'm not disappointed in having done it.

I posted an earlier comment a few weeks ago that I had cloudy vision post R cataract surgery done on July 2 that was complicated by a lens capsule tear and relocation of the implant to the sulcus ?spelling.  I still have cloudy vision and now a new problem developed about a week ago. I developed a R periorbital headache that progressed to fullness in my R ear and jaw pain.  The doctor on call put me back on steroid drops and overnight it went away.  I assumed that the eye and these symptoms were related.  On the steroid drops I had another milder episode about 3 days later, I saw my doctor and he took photos to determine if there was retinal edema, but there was no strong evidence of this from the photo.  He took me off of the steroid drops as I also have glaucoma and he did not want to create more problems.  Now today about 1 week after the last headache etc the symptoms are there again.  I am wondering if this can be related to the drops I am taking, a nonsteroidal called nevanac. Have you heard of these symptoms occurring in other patients? Also given the complication, do you think that the vision will ever improve and what possibilities could be causing this cloudiness?

Your problem is so dependent on the actual physical examination of the eye that I cannot comment.

Glaucoma can cause headaches as it it deals with the inter-occular pressure within the eye.  I personally don't have glaucoma, but I also get headaches and I feel it has to do with the cataract lenses so close to the eye.  I feel the vision is comprimised with everything so blown up.  My doctor raved about the cataract implants and I feel he was somewhat overrated in his exhuberance.  I got more out of the vitrectomy since I just learned that the cataract surgeon had to cut something that affected my tear ducts and now I have dry eye which also causes vision problems.  I also have a membrane growing from leftover cells he couldn't get out (microscopic).  My father-in-law had cataract replacement lenses done long ago and he didn't have any of these problems!

Thank you for answering my earlier question regarding vitrectomy for floaters.  I looked at the website for the floater questions.

Perhaps I could ask you just one more question.  Have you followed what happens down the road in later years after a person has had their floaters removed?  Just curious as to how they could see and how they were unhappy as you said?  Could you be more specific?

If a person has their natural lens then after the vitrectomy a cataract develops over time. It is possible for new floaters to develop OR they may be "cured". Again it is very uncommon to do a vitrectomy for "regular floaters".
