Which Monofocal lens for High Myopic -20..

Hello,
           Great Forum!...
As you see below...I am a very high myopic
Rgt –20 /+2.5 x  100  Left –20 /+1.25 x  95

Having worn soft contacts now for 25 yrs and spectacles ..(age 43) I am now considering Lens implants using CLEAR lens extraction method ..thus eliminating future cataract occuring...which in my case is inevitable ..sonner thn later ....prior to reading this forum i was tempted to have Multtifocal or toric Implant...having read the horror stories with regrd to Multifocals..I have decided the best lensses for me are monovision.  and if possible will hope for minor astigmatism correction ..at time of surgery using Limbal Relaxing Incisions.

This was also the suggestion from a family friend who happens to be an Eye surgeon.

However With such a vast choice of Lens manufacturers ....can anyone please suggest  there experiences with any particular brands of lenses, material type, size etc... which are better than others, and have proven to be reliable over a long study period.

Any other suggestions and advice will be very greatly appreciated.

Hi Jodie & JCH III MD
I have seen some very helpful replies form many people on this forum and yours and JCH III MD only underlines this more so...I am very!!! pleased you have had good results with yr implants...I have checked out the above site and I agree it does have a very good promotional video but as you mention having done quite some research there is very little between them and therefore i will disscuss with the
surgeon as suggested by JCH III MD...which lens he feels is best for him to work with in order to acheive best results.... At the moment I am still looking a little further into the risks asscoiated for high myopics having IOL surgery..but atleast the choice of lens is going to be correct for my needs .....again a BIG!! Thanks for all your helpful advice...

Kind regards
Indi

I doubt you would be able to tell the difference among the three.

There are 3 aspheric monofocal lenses (from AMO, Alcon and Baucsh & Lomb), and no credible evidence that one provides better vision than another (unless you are post-laser vision correction for farsightedness.)  Claims to the contrary seem to be hype from the manufacturers--I did a lot of research before my own cataract surgery.  (I got Alcon lenses because that was what my surgeon used.)

All three aspherics are FDA approved.   AMO and Alcon have both provided additional evidence of improved night vision.  In my case, anyway, this "improvement" is so subtle that it's totally undetectable.  Alcon and Bausch & Lomb lenses incorporate a feature (of questionable value) which allegedly protects the retina against age-related macular degeneration by filtrating blue or violet light, respectively.  (Just about all IOLs except Crystalens provide UV protection.)

But there's a clear winner in terms of which lens has the best website:  the AMO Tecnis.  You might want to check out the patient education video at www.tecnisiol.com, if you haven't already.

So all three aspheric IOLs are probably winners.  And you should really benefit from having limbal relaxing incisions to reduce/eliminate your astigmatism, which would blur your uncorrected vision with any lens.  You are very wise to do your research before making a decision.  Best wishes for excellent results!

Thank you kindly for your reply....yes....I have some vitreous gel degeneration..and am told this can increase complications...However my optomerist points out vitreous gel degeneration is bound to be the case as I am High Myopic..therefore not unusual otherwise i have very healthy eyes.
Also in your experience what percentage of people with high myopia suffer with retinal detachment during or post surgery, and can a surgeon detemine the risk with each individual prior to surgery.

With regard to lens I had narrowed down to the Tecnis, and Acrysof IQ-
both
Acrylic Aspheric...6.5mm if possible
From what I have read the Tecnis claims to have the best night vision and is FDA approved..and have also read good reports on Acrysof IQ...having good night vision...My concern is to have the best vision possible and have to narrow down from these two choices.
Howeveer you make an EXCELLENT point ...that individual surgeons will unnderstand ceartain lenses better than others and that may be one of the biggest factors.

Your further comments and help is highly appreciated....

There are many find monofocal IOLs on the market. Our practice use Tecnis aspheric IOLs. You should probably use the IOL your surgeon is most fond of. Many surgeons use their own individual formulas for determining IOL power and a unique K constant. If they switch to an IOL they don't use often it can throw off the calculations.

I am not trying to talk you out of the surgery but be sure you understand the risks especially retinal detachment.
