convergence insufficiency

i am 29 male suffering from  dry eye and allergic conjunctivitis for last 3 years. suffering from convergence insufficiency for last one year. no relief from orthoptics my near exophoria range from 10-16 prism diopter and distance about 4. started having diplopia often for last 2 months. cant read for more than 5 minutes , blurring occurs for distance also currently using 4 prism diopters base in in both eyes that is total 8 base in prism diopters for last one month. still suffering from headaches, horizontal diplopia, nausea, unable to read. is surgery an option whats the success rate
