converting eyeglass script to contact lens

My eye doctor gave me a contact lens script and said to speak to an optometrist to convert it to a contact lens script by the optometrist wont do it.  I have been wearing contacts for 10 years so I just need to know the power of the lenses to order.  My script for the glasses is:

OD -5.50 cyl -1.00 axis 4
OS -4.50 cyl -0.50 axis 6
base curve 8.6
diameter 14.00

What power contacts do i need?

I think that your optometrist would have you try out some sample contacts to determine what power worked best for you.  If you ordered -5.25 and -4.25 it would probably work fairly well, but it would not necessarily give you the best vision possible.  In my opinion, you'd be better off paying for the optometrist's exam, especially if the contact lens brand you've used previously has been phased out.

I am pretty sure Biomedics 38 has been phased out just recently...

I asked him specifically for a contact lens script and this is what he gave me.  I have been wearing biomedics 38 for literally 10 years, with a base curve of 8.6 and dia of 14.

If i order contacts with a strength of -5.25 for the right eye and -4.25 for the left, is that a bad thing to do?

I cannot do that for you becaus you are not my patient.  Someone needs to actually write you the proper contact lens prescription.  This is written incorrectly and  doesn't even have the brand of contact lens.  Call your "eye doctor" and have him write you the proper prescription.
