Nothing in medicine and surgery can be guaranteed. No surgeon can promise or guarantee a perfect result. No surgeon, no matter how skilled, gets 100% success.  Moreover eye muscle surgery (strabismus surgery or squint surgery) is very prone to under- and over- corrections. Second or even third operations are not uncommon.  Other complications like infection, bleeding, double vision, damage to vision could occur. You should clarify this with the surgeon. I believe you misunderstood what he/she said. If in fact the surgeon is offering a 100% success rate or guarantee, then I would consult with another strabismus surgeon.
