Can a Pheochrymocytoma not show up on urine, blood and or MIBG scan? Do you have to catch some Pheos in an active state or episode to detect?

There is no test that is 100% sensitive, meaning there is always a chance that tests can miss the disease.

The sensitivity is best with the blood metanephrines (99 percent) followed by urinary fractionated metanephrines (97 percent).  These tests should be considered if you want to ensure a pheochromocytoma isn't present.

Regarding imaging, this should normally be done after biochemical tests.  The MRI would be the most sensitive, but still not 100 percent.
