Migraine during orgasim

I have begun having excruciating headaches while having an orgasim.  This is a new development -- four consecutive episodes.  The headaches don't last long, but are pratically incapacitating. In fact, along with the pain inside my skull, I almost feel like my eyes are going to pop out of their sockets! Please know, I'm no sissy, having delivered each of my babies without painkillers. But, this kind of pain is new to me.  I am 46 and otherwise healthy.  Should I be concerned? If not, can you explain why this is happening?

The first thing is to ensure that it is indeed a migraine and differentiate it from a tension headache or something more ominous like a bleed or tumor.  An association with nausea and photophobia (sensitivity to bright light) can sometimes suggest a migraine.  

As for imaging tests, the most useful one would be a head CT, which can easily exclude some of the more serious causes like a tumor or bleed.  If negative, then you may want to consider treatment for migraines with a class of medications known as triptans (i.e. Imitrex).  

If the diagnosis continues to be non-revealing, I would consider a referral to a neurologist.
