Most of the initial tests have come back negative.  

A pinched nerve can be considered.  This can be evaluated with a nerve conduction study, and if positive, followed up with an MRI to rule out a herniated disk or disk bulge.  A referral to a physiatrist (rehabiliation MD) or orthopedist can be considered.  With the sensation of electrical shocks, you can consider a referral to a neurologist.

Many diseases can lead to increased shortness of breath.  A D-dimer blood test can look for blood clots, and followed up with a chest CT if positive.  Pulmonary function tests can look for obstructive lung disease.  If the lungs are clear, you can consider an echocardiogram to evaluate the heart.
