Do not know what is wrong with me, scared?

Hi my name is michael i am a 25 year old male.  About 2.5 years ago i started getting this pain on my lower left side of my back.  Then i started getting these weird headaches almost like i have something bad in my head and i need it to be scraped out.  I get really bad joint pain.  Really bad back pain, sometimes chest pain, but only on one side, either the left or right.  I have blood in my stool.  i get like these prickly sensations on my penis.  I get anal itch.  I itch on my scalp, underarms, side of my left foot.  I have an infection in between my toes on my left foot.  Pain shoots up my left leg.  I work as a painter and sometimes i feel really bad pain in my entire back, like my back is going to collapse.  I always have to go to the bathroom, and my penis sometimes smells fishy.  My urine has a really bad smell.  My memory is horrible, i used to have a pretty good memory.  Its hard to concentrate, i am always worrying.  I just went back to school part time after getting my associates degree part time and i dont feel even close to a hundred percent.  I used to take alot of ecstacy.  I had unprotected sex numerous times.  I got tested for stds and nothing came back.  Are these prickly sensations parenthesias?  I have been to the doctors over and over, mostly the health center by my house, and i hate that place, i dont have money for a good doctor.  The lady at the health center said i had anxiety problems, but these symptoms have been present for 2.5 years this cannot be anxiety.  I got tested for lymes disease negative, My doctor at the health center said something about a spinal cord disorder, spina bifida, she said its an opening in the spin, could that cause these symptoms, but i hate that place.  They are so quick to diagnose you, i had high blood sugar they said i had diabeties.  Then i never went back, but these problems still pesist,  I just want to have a clear head again, with no pain.  Where do i go from here.  The last time i went to the health center i had a list with all of the symptoms i listed for you and the lady wouldnt even read it, she looked at me like i was crazy, but i never felt this way before.  Can you point me in a direction that i can narrow down my diagnosis.  Could this be an std, back disease, diebetes, if you could narrow this down for me maybe i can solve my problems.  Thank you for your time and i pray to god that you can help.
Michael

Difficult to say without evaluation.  There are many causes for the symptoms you have described.  This can include infection, musculoskeletal strain, neurologic disease, or various GI diseases.  The only way to tell would be to systematically test for these options.

If diabetes is uncontrolled, this can lead to neurological and GI symptoms.  Obtaining a repeat fasting blood sugar test can be considered.

Neurologic disease can be evaluated with an MRI of the spine, as well as possible electrodiagnostic testing.

GI disease can be considered, with the rectal bleeding, with a flexible sigmoidoscopy or colonoscopy.  An abdominal CT scan can also be done to rule out inflammatory bowel disease or diverticulitis.

These options can be discussed with your personal physician, or in conjunction with the appropriate specialist.

Followup with your personal physician is essential.
