A cyst can be considered - however, this is normally present during the fat layer. Infection of the cyst can be considered if the area is red.
If the lesion is further down, you may want to inquire about imaging with an ultrasound for further evaluation.
A more definitive diagnosis can be obtained by removing the lesion surgically, or via biopsy.
