MRI Impression

I had emergency surgery 3 years ago for a herniated disc that had ruptured and fragmented causing numbness of my right leg and foot.  Recently my back is troubling me again.  I had an MRI done and need help with the impression as to the severity of the report.  Broad disc bulg with right paracentral to lateral disc protusion in combination with some scarring at the level L5-S1 causing some mass-effect upon the thecal sac and the S1 nerve root on the right.  No definite spinal stenosis seen at this level.

Degenerative disc changes at L4-5 and L5-S1, greatest at L5-S1.

Mild broad disc bulge at L4-5 without evidence of focal disc protrusion, spinal stenosis or foraminal stenosis.

There is some deformity in the contour of the ventral surface of the thecal sac at this level.

What does this mean?  What are my treatment options?  My Doctor will not call me regarding my report - I had to pick it up at the hospital.  I am in severe pain and continually locking up where I cannot walk.
