shoulder pain

Hi I have a very sharp pain between the shoulders slightly to the right  just about where the neck pivots ,the more my head goes down the sharper the pain. I  get this pain mainly when I am cold or very tired and been on my feet for a few hours. I am a 52 year old male 6ft 1 and weigh approx 95 kg and also suffer with high blood pressure and also a smoker  ...hope you can help as I fined working outside difficult so it is affecting my employment  thank pete_denno
