Epididymial hypoechoeic solid mass

Hi doctors, am 26 years old and since two weeks am sufering of the presence of epididymial hypoechoeic solid mass (1.8*1.6*1.4)cm =2.2ml  at the tail of my right testis and a small cystic mass (1.0*0.6*0.7) at the epididymis head. The first doctor i met send me to the surgery clinic for surgical operation but i refered to the second doctor who just give me antibiotic drugs that am taking now. am really afraid because the tail of testis is still very strong and sometime painthull. what can be the causes of such diseases and what can be the consequences of surgical operation on my testis? am really affected morally because i  don't have child and in my place, a man must have a child to be integrated in the society. can these diseases affect my fertility?
