IVF after failed vasectomy reversal

We are looking into IVF after a failed attempt four years ago, at a vasectomy reversal. My question is two parts...First regarding insurance coverage.. I have BC/BS IL HMO. I know they cover fertility treatments but would I be denied based on the fact that we initially underwent voluntary sterilization? Has anyone gotten an approval for IVF in this situation? As far as the medical side of things..we were going to look into doing sperm aspiration/ICSI, IVF. After having had a vasectomy for about 9 years, along with a failed reversal, what are our chances? Is there any other procedure that might be better? Can IUI be used with sperm aspiration? Thanks in advance for any help/suggestions.
