Disjunction is a very common result of mitosis (cell-splitting). When the right amount of chromosomes don't go into each new cell, disjunction occurs. As an example - the human cell has 46 chromosomes - they double to 92 and then split into two cells with 46 chromosomes each - copies of the original cell. If 30 cells go in one cell and 62 cells go into another cell - disjunction. Then the cells would either not be able to reproduce anymore and stop cell division (if it happens early on in development) or end up in a very serious condition.
