fetal heart rate of 143 and still miscarried

This is my second miscarriage and at 7 weeks 6 days the baby's heart rate was 143 and doctor seemed pleased with this. at 12 week sonogram baby measured 7 weeks and heart beat stopped. what could be the cause? Everything before this point was perfect: hormone levels, etc except that he refused to test for progesterone, which at the time seemed logical. but now I fear that was the cause (low progesterone). What to do?
