how to approach fertility issues with doctor

I have my annual exam on thursday and I want to approach the issue of fertility but I don't know what to say or expect. We have a 3 y/o DD and I have been off BC for 2 yrs. No intervention was needed for DD except  I had had a  HSG done a couple of months prior for an ectopic(had the left tube removed). I have pelvic congestion syndrome and the pain from that peaks at about time for O so I know that our timing of BD is off some months but I'm not sure how long we should wait before thinking there may be an issue. We are 28 y/o and healthy. Also I think I have another ovarian cyst, (I've had RLQ pain for 1 1/2 weeks (HCG is negative abd u/s is scheduled for friday)).
