elevated anticardiolipin antibodies

by nikkicole4, Jun 01, 2007 12:00AM
I am 23 years old and have had 3 spontaneous abortions since 9/06.  My Dr. just did thrombophilia testing.  Anticardiolipin (IgM) antibodies were elevated to 13 which my dr. said was in between negative and postive. Also protein C was high 154.  My Dr. does not know what to do next with these results and is referring me to a RE.  Should I be staring aspirin or heparin therapy? No other clotting disorders were found. Thank you.

Hi there - based on the results you have offered, there is no reason or indication to take heparin. A high Protein C is OK - it is protein C deficiency that may be associated with recurrent pregnancy loss. You anticardiolipin IgM in my opinion is not elevated, and making the diagnosis of what is called 'antiphospholipid syndrome' requires two elevated values at least 6 weeks apart.

As for aspirin, low dose ASA (81 mg tablets) once a day may be helpful simply from the standpoint of improving uterine blood flow and therefore implantation, so my thoughts are that low dose ASA might help, won't hurt, and could be taken. Make sure it is low dose though, too high a dose (the regular adult strength) can, in theory, have a negative effect.

After three miscarriages, I agree it is a good idea to see and REI. Most of the time everything tests out normal and you have a successful pregnancy the fourth time, but it is definitely worth looking for any contributing factors to the miscarriages that could be treatable.
