TTC for 14 months after Mirena with Low Progestrone and High Insulin

We (me 25 and hubby 28) have been ttc #2 for 14 months since 07/08 and haven't had any luck :(  We live in a rural area and the hospital/dr office we use hasa a policy that you try for 12 months before using fertility treatment.  We conceived our first without ttc, we had been together for 6 years and never used birth control, just home methods.  So I am unsure if I had a problem before or we were less careful one night.  After having my son I went on the Mirena for 2 1/2 years, I had breasfed him to 10 months so I never had a period after the 1st post birth.  After having the mirena removed I had 2 regular periods and then stopped completely, finally got my dr. to prescribe Provera to bring on the periods and it helped.  After 12 months of ttc  I was given 5 days of 50 mg of clomid and my dr. said that I had LOW progestrone levels with a .9 and high insulin but wasn't worried about it.  After not getting pg that cycle I went back and had to see another Dr. as mine was out and the new one checked my levels I was 1.0 on Progestrone (with 1 month of 50 clomid) and my insulin was 74 which she said is high and that 10 is what would be good.  She put me on 1000mg of Metformin per day and another 5 day cycle of 50 clomid.  I have been charting and it does seem like this time I did ovulate and my temps have stayed higher than ever before, but still not extremely high.  The last cyle my temp went up 1 day and stayed relativily high for 3 days and then dropped back down.  This time I started my cycle with a 96.7 and peaked at a 97.4 on the 17th day then my temps were 97.2, 97.1 and 97.1.  Has anyone had anything like this?  My Dr. doesn't think that the Mirena had anything to do with my problems but I can't help but wonder?  I haven't been diagnosed with anything just being treated for the symptoms.  Please give me any thought, insight, info or anything..  Please...
