progesterone needed and when

I am 41 and ttc a fourth baby. I have had three successful pregnancies all 3 healthy boys ages 15, 11, 3. I had to use progesterone for the last two but only took the suppositories after the first blood test. I have miscarried between my first two boys, once before my third and four times after my third. I would call the recent five miscarriages the disappearing line ones here I get a positive result and then within a week or even a day or two I will start staining and then no line on the ept tests. After my third child I have had a new doctor and she believes in only giving progesterone once my numbers on blood work show a viable pregnancy. I am concerned that by waiting until then, I will not be able to maintain the pregnancy. I have read online that sometimes starting progesterone after ovulation can prevent these "chemical" pregnancies or "instantaneous abortions". I have conceived each time I have tried every single month that my husband and I have decided to go for it. So I know conceiving is not the issue. ALso I was told during my second pregancy I had a reproductive age that was 7 years younger than I am. I feel confident that I can conceive as I still ovulate and get regular periods but I seem to have issued keeping it going. Any suggestions? Should I start cream on my own? SHould I find another doctor?
