I HAVE RECENTLY  APPLIED FOR INCAPITITY BENEFIT, AS I HAVE FIRBROMYALIA ,I HAVE BEEN TOLD I CAN GET IT BUT I I HAVE TO FILL IN A FORM AND HAVE A MED4 FORM FROM THE DOCTORS,CAN ANYONE HELP ME,WHAT WOULD THE DOCTOR PUT ON THE FORM
.I AM GETTING STRESSED OUT ABOUT IT ,DO I HAVE THE CHANCE TO SAY HOW I AM ON A DAY TO DAY BASIS , THANKYOU IF ANYONE CAN HELP

the form you can also ask for is ADA the Americans with Disabilities Act.....it covers alot of things with work and special needs...if you can not work at all...contact your local social security office and they can direct you as to how to get benefits.
