Abductor hallucis treatment?

For about 15 years, I have had this pain off/on in my abductor hallucis...I think...  I google searched the different foot muscle parts and I honestly believe this is the issue.  The pain comes during the night (during sleep); it's like a golf ball; muscle spasm.  And the pain comes when I wear rental footwear eg. ski boots, snowboarding boats, ice skates.  I have seen 3 Podiatrists and they believe it is related to pronation.  And one doctor evey recommended surgery.  What are your thoughts?

do you have wide feet?  if you experience pain with rental shoes, it could mean that you are irritating the medial portion of the foot due to ill fitting shoes.
if you continue to experience this pain with regular shoe wear (non-rentals), orthotics can help...especially if you do have pronation.
i wouldn't have surgery unless the conservative methods don't help.

Thank you!  Wide feet - no; my measurement is just a D.  In regards to orthotics, what is the best way in getting your feet sized up?  i.e. foam, using the map, or where the podiatrist moulds your feet?
