Heart burns and pains

I have heart burn with the burning sensation spreading all over the stomach. I also have bloated stomach. I frequently belch, experience some mild pains swallowing but most irritating of all I have back pains just behind where I experience the burns. I also have pains on the shoulders and some times the arms.
