A baby has no way of communicating other than crying...so if an infant has distress with eating....he learns no to ask for it....by not crying.
See if a wedge pillow under his matress may help in elevating him and reduce the reflux affecting him during sleep.
Keep him in lose fiiting clothes . U did not mention if he is breast fed or on formula.
I am not a dr so I can not advise on th esleep apnea or the siezures.
Was ur nephew checked for allergies?
As to the brain stem statement by his dr...I can not comment.
