right upper cuadrant pain for looong time.....

I have been having this hipocondriatic, depresive right upper cuadrant pain like for 2 years, maybe more, some months i can go to the hospital 1 time a week, sonograms, x-rays, ct scans, bloodwork for pancreas, liver ect, everything comes back normal no gallbladder stones, well distended, everything unremarkable, it gives me nausea and i feel sick, extremely depresive and stops me from doing my daily routine, im 31, male a little bit overweight but i do my workout. actually all my live i have done exercise. im very desperate cause doctors said theres nothing wrong, but im in pain and without a diagnostic its very depresive. endoscopy and colonoscopy have been done too, nothing. any help would be greatly  recived..........

Hello and hope you are doing well.

Ultrasound and CT scan may show structural abnormalities of the gall bladder, but for a functional assessment, you may have to get a HIDA scan done. The other causes could be gastritis, peptic ulcer disease and GERD. Kidney disorders too produce colicky and sharp pain. Sometimes anxiety with flatulence can cause this pain. You will need to consult your primary care physician for proper evaluation of pain and therapy

hi dr Anitha, tanx for your answer now we are in january and im WORSE the pain and nausea are there 24/7, from the time i first wrote to today i have been in er like 10 times, yes 10 and went to my primary like 3 or 4 times. the thing is blood work for kidney and liver, pancreas everything is just perfect, i had another ct this time with contrast by mouth and iv......unremarkable pretty detailed study by the way, another ultrasound, plain x-rays wow nothing, i cant eat i have lost like 15  pounds since this begun i feel like something is serious unatended but doctors just say David you need vacations ur perfect. and im not, the only thing i know is that 2 years ago my gallbladder ejection fraction was 35% can this be worse by now i have not done the HIDA yet........im beggining to think this could kill me :(

Hello and hope you are doing well.

CCK HIDA or HIDA with ejection fraction (EF) is seen to check for gall bladder ejection. If gallbladder ejection fraction is below 35% is considered to be gallbladder dyskinesia.

If there is gall bladder dyskinesia, you should be having fatty food intolerance and sometimes pain in the right upper quadrant if there is evidence of gall stones or inflammation of the gall bladder. If due to non-functionality of the gall bladder, there is stasis of bile, it could predispose to gall stone formation. You could request for a repeat HIDA scan.

wow im in shock!!! we are in february and im still sick !! now the gi told me that all my studies are unremarkable and there's nothing wrong with my galbladder
he told me i have 2 problems, muscular problem in my right side and ibs...
whaaaaat??  i told him the pain worsens with food and nausea dont let me live.... can ibs be really doing this... he's going to do an endoscopy and colonoscopy  i feel hopeless..... i live in Puerto Rico  maybe medicine here its not good enough...my primary and now my gi are telling me this could be my mind and they want me to see a phsy, i fear the worse or maybe im really paranoid about this, i have lost weight i have been a very healthy guy all my life thats why i insist theres something wrong because its not normal to feel sick every day of your life..............

IBS is a diagnosis of exclusion. If all the tests to rule out other disorders are done and found to be negative, then the diagnosis of IBS is made.

As you continue to be investigated, try life style measures they help to reduce the severity of symptoms. Stop becoming over anxious and practice relaxation techniques. Exercise, go for long relaxed walks. Avoid those foods which you feel are increasing your symptoms. Stop smoking, eliminate alcohol consumption, and avoid caffeinated beverages and spicy food.

Hi there, we have a very similar problem. In september 2010, i was admitted to hospital with gallbladder area pain. I'd previously seen my gp twice. It comes in waves and sometimes it's so frequent! The waves of pain/spasms are so bad at the moment. I had a barium xray and biopsies with camera on my bowel which showed nothing. Then i was told the camera doesnt go up that far so what on earth was the point in me going through all that? Anyway, i've had a gallbladder scan and it showed....nothing!!! That was in the sept 2010. My gp said that people can have tiny little crystals or sand like size gallstones which are not detetcted on scans and that i may have this.
Anyway, i am about to call the drs as its got that bad and i want another scan i think as its been well over a year with no improvement, in fact worsening.
I don't have the sickness or frequent nausea as you do but have felt a little nauseus before, whetehr related to this i don't know. My old gp said i'd fly off the bed if she touched the area where my gallbladder was if i had gallstones but my new gp said this isn't true unless they are causing a lot of irritation already?
My mother had her gallbladder out 2 years ago and my sister is only 25 and has 3 large gallstones. My sister is very overweight and they told her that her diet was probably mostly to blame.
I used to run or visit the gym 4-5 times a week, i don't smoke (have a long time ago) and i don't drink except for maybe a shandy or glass of wine perhaps every 3 months or so.  I do have other medical problems, i have hypoparathyroidism (rare disease affecting calcium and vit d levels) which causes a lot of symptoms which are under control generally as long my cal level is stable. It doesn't cause this pain though and after suffering ibs for 9 years this upper right pain, to me, does not feel ibs related. What is odd though, i see people write it always starts after food....mine is there sometimes when i wake up before i've eaten or drank anything. It gets worse most of the time after food and since its been at its worst the past few days ive had reflux too.

Perhaps ask your gp about the tiny stones thing? I know your frustration, and i was told for a long time i had nothing wrong with me prior to the hypo pth diagnosis because the symptoms are invisible, and my gp said 'go away and stop thinking you are ill and the symptoms will go'. She was horrid. I've basicly been told the same about this, nothing to see, nothign to prove theres somethign wrong so therefore the pain does not exist. Well its not in my head, i'm in a lot of pain.
