Follow-up on Osmolality tes


  The results are in! It seems I have a secretory diarreah (diarrhea) and my sodium is high. What treatment options are available to control this. I have already tried anti-diarreals(imodium (immodium),lomotil) with no success. My doctor is suggesting mucho glucose. I am currently "maxed-out" with glucose in my TPN. Will oral glucose make that much of a difference? I'm looking to decrease my output from 25 times a day to something more reasonable. Thirty months of this nonsense is a good place to stop. Secretory diarrhea is the result of the release of a compound that acts on the intestinal mucosa to cause ti to secrete more ions and water and to absorb less of these constituents.  Pancreatic tumors, lung tumors and carcinoid tumors are among the common causes for secretory diarrhea.  As a first step you should have blood tests for serum VIP level, gastrin, pancreatic polypeptide.  A 24 hr urine test for 5-HIAA may also be needed.
Therapy is usually directed at the tumor.  If a tumor can not be found or is unresponsive to therapy or there is no detectable cause for the secretory diarrhea, we will treat patients with Octreotide in an attempt to control the diarrhea.
