Gastroparesis-nausea control?


  My 17 yr.old son was diagnosed with gastroparesis last week after
  an EGD showed food in stomach.  He has missed 4 wks of school now
  because the nausea and abdominal interferes. The dr. put him on
  Reglan 2x day and Prevacid 2x. His GP had him on Prevacid for
  reflux which just started working & propulsid  which caused
  fast heartbeat and twitching. He has been on Reglan 7 days and
  the nausea and abdominal pain came back after several days. If we
  could control the nausea he feels like he could get back into school, as he's missing out on everything. I called the gastro-
  specialist who diagnosed g/par. His nurse says that if it gets worse to call back. She realy wasn't interested in his diet.  Since he was doing better up until he ate 2 roast beef sandwiches
  I thought this was important. Should we expect better results this fast or is this normal for him to stay nauseated? He does not vomit or have diarrea (diarrhea), but does have reflux. We cut out fats,
  milk and meat, which does not leave much for a growing boy.  I'm
  sorry to go on so long, but we both are so stressed out because he missed all last semester with a "mono like virus".  We thought
  he was doing great this summer and beginning of school. If you could tell us of anything that might help with nausea or if we just need to have patience we would appreciate it.  The dr. said
  he should be able to go to school w/this medicine.  Thank you.

Your son shouuld avoid fatty foods as they slow gastric emptying even in normal subjects.  The dosage of Reglan that your son receives is low.  Usually, we give 10 mg pills four times perday not twice.  It is possible that an increased dosage will improve his problem.  Finally, the time between starting treatment and seeing an improvement in symptoms can vary but if there has not been substantial, persistent improvement after 7-10 days, then an increase in dosage is warranted.
