Hiatus Hernia and Gerd

I was diagnosied with Gerd and Hiatus Hernia in 1995.  Since that
time I was treated with prilosec and tagament which have given ver
little relief. I tested positive for H.Pylori and was treated
for that.  Since that time I have not been in to see my physician.
I do have an appointment on the 13th of October.  My concern is
the low grade acky pain(on the left side) sometimes sharp thru my
chest, back and shoulder pain, the inability of sleeping on my left
side, nocturnal reflux  that wakes me up coughing, heartburn with
everything I eat, and the worry about cancer.  The only testing I
have had is a Upper GI, this is when they told me I have a hernia
and Gerd.  I have not had any weight loss, and heart disease
does run high in my family.  I've read about Barrett's esophagus.
I'm very concerned and very tried of feeling bad everytime I eat
or lay down.  Would very much appreciate your opinions and
questions I should ask my physician. Thank you
