Is this pain coming from the colon?

Is this pain coming from the colon?

I am a 38yr old female. 5' 11", 160 lbs, good health overall (well until recently). Hypothryroid.

Was told I could possibly have IC (during the internal exam the bladder was tender and reproduced painful sex pain). Very last urine culture 3 weeks ago turned up a low bacteria count so I was put on Cipro for one month. However...my docs are not convinced its IC due to no nocturia nor or frequency (average 6-7 times with strong stream and volume). I was told pain in my lower back and on my left side (lower pelvic/ab area) is colon. The pain is burning and throbbing. Pain worsens as the day progresses. It seems to correlate with food/beverage intake (within 30 minutes or so) although I have been trying to watch what I eat and drink (no spices, caffiene or alcohol). Sometimes feel pain in the lower left abdomen during intercourse.

I am confused because the pain of IC and IBS can be similar and brought on by the same things and that they often go together. The past 3-4 weeks I keep waking up with extreme flatulence which goes away as the day goes on. I feel constant "bubbles" breaking throughout my body (especially rectally and upper abs). Was told to take Flora Q during the Cipro. It was around this time the gas kicked in so I don't know if it's related. Have an urge to urinate but not going more often. I have mild constipation on and off but no diarrhea. Stools are always solid but very pale.

I was told if it is colon related it can also cause the bladder pain and that may answer all my questions. Can colon problems cause the type of pain I am feeling? Especially in regards to the bladder and sex?

Any advice would be appreciated.

Tests done (all normal):
- Ultrasound and CT Scan of pelvic/abs (w and w/o contrast)
- STD and urine cultures
- Cystoscopy in office

I agree with the workup thus far.

To further evaluate the colon, I would consider a colonoscopy.  This can rule out inflammatory bowel disease, as well as any anatomical masses or strictures of the colon.

Pale stools can be due to blockages in the biliary ducts.  The normal ultrasound is reassuring, however, I would also obtain liver enzymes to check the bilirubin levels.  An MRCP can be considered to further image the biliary tree.

These options can be discussed with your personal physician or GI consultant.

Followup with your personal physician is essential.
