What you are describing, may be known as a globus sensation - defined as the feeling of something in the throat without problems swallowing.  You have had a very thorough GI workup including upper endoscopy, esophageal manometry and 24-hr pH test.  This pretty much rules out GERD or any other GI cause.

The next step would be an ENT referral to evaluate the throat and the area at the back of the mouth.  A laryngoscope can be done to evaluate this and this would be my next recommendation.
