More test results...What now?

  Hello again-
I submitted a question a month or so ago about my fiancee and his elevated liver enzymes with negative hepatitis screens.  Since this time he has been tested again with the same results - however he also had an ultrasound of his liver and his doctor (Internal Medicine/Family Practice) believes it to be fatty liver.  He is only 31 years old and is not/never has been a heavy drinker.  In fact, he did not drink at all until several years ago.  He has also obtained blood test results from 5 years ago and found his enzymes to be elevated at that time too (the doctor neglected to mention this to him).
My question is this:  after doing some research, causes of fatty liver appear to be obesity (he is 6', 170 - never been overweight) and heavy drinking (already noted above), and diabetes.  His blood tests also reveal elevated triglicerides (triglycerides) and high HDL cholesterol and his father is a diabetic.  Should we look into this possibility further?  He has an appointment in 3 months (HMO) with a gastroenterologist, but should he go back to his PCP to get tested for diabetes as well?  Please help!

Dear Kristy,
The high HDL is the greatest risk to his health.  You should see your primary doctor to arrange for treatment of the cholesterol.  With appropriate diet and medications the cholesterol can be reduced and the triglycerides controlled resulting in a reduced risk of heart disease and improvement in the fatty liver.
