: My fiance has been having fairly acute abdominal pain for the last few months.  This generally occurs after sex (although he says if he doesn't put too much pressure on the left side during intercourse it isn't as bad).  He dates the condition back a year.  He believes it may have occured after an intense abdominal work out before which he drank a large quantity of juice(dumb, we know).  That evening he was very nauseous and spent most of the night vomiting.  
He is in excellent physical condition so it's possible that that he was able to go a year without too much difficulty.  Now, however, he is experiencing pain.  He can no longer eat as much at one sitting without discomfort.  Also - after sex there is what feels to be a protrusion into the left testicle which disipates over time.  He says that it feels like something is "snapping back into place" when the pain occurs.  Based on this we went to a hospital that specializes in hernias - believing this to be an inguinal hernia.  They haven't a clue what it is!  My fiance is foolishly afraid of doctors and I'd like to have an idea what might be up before I talk him into going somewhere else.  
Any thoughts you might have would be very much appreciated.

Dear Lisa,
When I read your letter, my initial thought was that your fiance had
an inguinal hernia possibly a varicoele.  In view of the history of decreased appeteite, your fiance should see hs physician to obtain a complete evaluation of the GI tract, even though it is unlikely that there is a connection betwen the lss of appetite and the pain with sexual relations.
