Rectal Bleeding

My sister-in-law is only 23 years old and for the past three years she has had considerable bleeding during her bowel movements.  By considerable I mean that the blood is in the toilet, not just on paper...
She also wakes up every morning with very painful stomach cramps which do not go away until she has at least three bowel movements.  
She doesn't seem too concerned, but I am worried about her.  Maybe with more information she will look into this problem further.
