TPN and elevated LFTs

Subject: TPN and elevated LFTs

Forum: The Gastroenterology and Liver Diseases Forum

I have posted a few questions here before.  I am a 27 yof with
gastroparesis and I have been maintained on TPN for a little over
a year now.  I currently have a j-tube and am progressing very
slowly with the feedings (30ml/hr for 2-4 hrs/day).  There is
some question about whether or not I have intstinal dysmotility
in addition to the gastroparesis.  My LFTs have been steadily
increasing over the last few weeks.  I have never had liver
disease and we are assuming the increase is related to the TPN.
Most recently my ALT is 5 times the upper limit of normal and my
AST is 3 times the upper limit of normal.  My ALP is mildly
elevated and GGT and bili are still normal.  I know that this
is a well recognized complication of long-term TPN therapy.
What is usually done for this?  Is it just a watch and wait game
or is there any recommended treatment (other than discontinuance
of TPN)?  Thank you for your time.
