Young person with liver, biliary problem please help


  My son had a cholestectomy at age 11. He is 16 now. He still has intermittent attacks with varing degrees of illness, pain and lab results. It typically follows this path fatigue-->appetitie loss--->pain attack---->tender abdomen--->relief of symptoms until another attack...sometimes soon, soometimes months later. The pain is very severe, he sweats, cannot talk..it lasts anywhere from a few minutes to hours. (the longest period of pain was about 12 hours) WE have had DX's of everything from possible intrahepatic stones, pancreatitis, SOD, cholangitis, hepatitis, etc. etc. You can understand our frustration. 7 Stones/ chemistry was 3%cholesteral, 4%calcium,48%mixed bile pigments,45% calcium phosphate. From what I have red these are the type that can cause intrahepatic stone formation. Is this true? My son is scheduled to see another specialist who suspects SOD but thinks there is a possibility of small stones forming intrahepatically that are too small to see on xray. Is this possible? He had an ERCP in 1996 during an asymptomatic period..it was normal? I believe they are going to do another ERCP with SOM to check for SOD. When my son was an infant and toddler he had periods of clay colored stools, diarreah (diarrhea) etc., In middle childhood he had some bouts of abdominal pain and fever that was attributed to the flu. So I think this problem has been life long. Currently his ALT and AST are elevated(3-4X). Labs have sometimes returned to normal between attacks. ALk phosphatase, GGT, Bili,LDH, PT have also been elevated during other episodes. He has some attacks that come out of nowhere and some that happen after weeks of illness. He has had tests for wilsons, pyruvate,heavy metals, HEP A,B,C. All clear. My father has HH but my sons ferritin level was below normal 2 years ago, so this is not an issue. He has never had a liver biopsy.  I guess my questionis this..do you have any insight regarding my son. I realize you cannot diagnose but perhaps you could comment. thank you so much for your time, F. Thompson

I am afraid that I do not have any new diagnoses for you to consider.  My omnly suggestion would be to consider a liver biopsy if there are abnormal liver tests.  This information may accelerate making a diagnosis.
