Abdominal Discomfort

by RiversAir, Oct 13, 2006 12:00AM
Hello,

I am a 25 year old white Male, 165 lbs, with no prior medical problems. About three weeks ago, I started to have a "discomfort" not a pain, directly above my navel. It came upon all at once. I have no nausea, loss of appetite, diarrhea, vomiting, fatigue, etc. The feeling is not painful, it does not wake me up or prohibit me from doing anything. It is more annoying than anything. Prior to the feeling starting, the only drugs I took were Naproxen and PeptoBismal. I probably took Naproxen 2 or 3 capsules per headache, and PeptoBismal once or twice a week. Sometimes I had alcohol with these medicines. I was a large tea drinker.

Anyway, after the pain started I stopped taking all medicines, alcohol, tea, and now only drink water. When I wake up, I usually dont feel the feeling. I do not feel it during the night, if I wake up to go to the bathroom, etc. Sometimes I feel it all day. Sometimes I do not feel it until after I eat dinner.  Three of the days I did not feel it at all. Most times it feels worse after I eat a large meal, but not always. It seems to have gotten slightly better during the three weeks. The first week or so, it was all the time. The feeling can best be described as a slight bloating, sometimes with a very slight burning sensation. I do have a lot of stress professional and with my personal relationships, and I worry very frequently!

The only other symptom is yellow stool. It probably has a more foul odor than usual as well. I however, have no blood in the stool as the doctor tested that. I also had a urialysis which came back fine. Maalox, Pepcid, Zantac, and Tums do not make a difference in the feeling.

I got blood work done (all normal):

Glucose: 95              CALC OSMOL: 282
BUN: 14                  Calcium: 10.6
Creatinine: 1            T Bili: .9
BUN/CREAT: 14            ALK Phos: 85
Sodium: 141              SGPT (ALT) 29
K+ Serum: 4              SGOT (AST) 25
Chloride: 104            Total Protein: 8
C02 Content: 28          Albumin: 5.9
ANION Gap: 9             Globulin: 2.1   Ratio: 2.8
TSH: 2.6    Vitamin B12: 385
Folate: 13.4        Hemoglobin A1c: 4.9
WBC: 6, RBC: 5.72, HGB: 17.8, HCT:, 51.1, MCV: 89.3, MCH: 31.1, MCHC: 34.8, RDW: 12.8, PLT: 280, MPV: 8.5, NeutroPhil: 4.4,
Lymphocite: 1.2, Monocyte: .4, Eosinophil and basophil 0, Lymphocite %: 19.8, Neutrophil %: 72.9, Monocyte %: 7, Eosinophil %: .1, Basophil % .2

I then had an complete abdominal ultrasound. The ultrasound came back fine, except for a kidney stone in my right kidney. 4mm. They said it may or may not be causing the feeling. I have  an appointment with a Gastronentologist on Tuesday.

1. What do you think could be causing the discomfort? Ulcer?
2. What questions should I ask the doctor?
3. What test do you recommend next?
4. Do you think the kidney stone could cause abdominal discomfort and a bowel change?

To answer your questions:
1) I would investigate various upper GI conditions.  This would include an upper GI series or upper endoscopy.

2) The aforementioned tests should be discussed.  Blood tests can also be considered to look for a bacteria known as H Pylori - associated with ulcers and inflammation.

3) An upper endoscopy - to comprehensively look for ulcers, inflammation, GERD or other upper GI disorders.

4) Possible, but kidney stones present with blood in the urine and flank pain.  I would investigate the upper GI tract.  If testing remains negative, an abdominal CT scan can be considered.

Followup with your personal physician is essential.

I am 23 year old male,
please help this likes to make me crazy.
Though your symptoms is different from me we have some similarities.
Before 3 years i start to have these signs; want to vomit & weaknes during morning
wakeup, loss in appitite specially during morning and if i slept during day. This increases from time to time for 2 years and developed high constipation.
Then i went to a specialist doctor last year, after checking my blood, stool&urine he told me i am H pylori positive. As a treatment he gave me the antibiotics ammoxaxilin, calathromycin, omoprazole(lucid) for 14 days.i didn't take the ammoxaxilin  correctly case i do afraid of these medicine after taking at one time before for another treatment , but i did take the two antibiotics.it seem better during taking the medicine but it returns after all the medicine is taken.then i went back to the dr, he did nothing and told me he can not check for hpylori now it will be only checked after 2 years.
Though life was difficult i did live for 10 monthes by  wakeup around 5a.m with disturbed stomach & drinking water all this to make my engineering study.During my final exam tests of the year i again did have high vomiting during morning specially when i am very hungry during the night .
Then i went to another dr,after blood test he told hpylori positive & give the same treatment as above for 14 days i did  take it (except the ammoxaxilin). But there is no change then i back to the doctor after a month he did check d-test for hpylori and told negative result. And gave me ranitidne & worms i took these but there is no change still.Though i did try all this with financial problems of the expensive medicines &lab tests nothing is changed.I can not take more testes like urea breathtest, endoscopy, biopsy becase they are not available here.
ANY ONE WHO HAS THE SAME SYMPTOMS LIKE  ME  OR ANY WHO CAN GIVE ME ADVICE IS VERY APPRECIABLE
My current situation is
1)I do have vomiting during morning
2)high constipation(if feaces gose off on time my appitite increase)
3)burning at my back side
4)very weak feeling in my legs starting morning up to late morninig
5)i don't have any reflux acid
6)i don't have burninig or pain in my stomach
