Another anal sphyncerotomy?

Three years ago I underwent a sphyncterotomy after many painful years of having numerous fissures.  It had come to a point where I would have many fissures at once and would cry everyime I went to the bathroom.  The past 3 years I have done great and have not had any fissures, but now I seem to have gotten one again.  I have gained and lost 45 lbs since I got the surgery and I dont know if the weight loss could have triggered the problem again, I dont have any rough stools and my diet is better than ever.  Is it possible to get another sphyncterotomy after already getting one before?  Is there another permanent solution for fissures?  I have had fissures since I was a kid, I just dont want to be in pain anymore.  Thanks in advance.
