I am sorry you are having these problems. I imagine the pain and discomfort you have are maybe causing your muscles to seize up.
I suppose you could ask the doctor if you'll need to come back for another injection and how long that interval might be. I would also ask him if the shot would in some way affect the control you have over when you have to pass waste. Maybe he's thinking a one-time shot would help your fissures heal, since the muscles wouldn't spasm anymore.
I have heard in this forum about people getting those fissures operated on, but I'm not sure about the success of those things.
I have to assume that you have tried keeping the area lubricated with KY jelly, you put it on after showers, and if you pass waste, to clean the area and apply more jell. I also have to assume your bowel movements are normal, that is, you are eating enough fiber type foods to stay regular, so that when you have a movement, it does not stretch the anal area too much by having, for example, a hard movement.
