Constant anal pain

About 3 weeks ago, I had a really hardened stool, with a really sharp pain.  I am assuming at that time that it created an anal fissure.  In recent days, have begun to have days with constant pain/throbbing whenever sitting around for more than 5 minutes or so....went to the doctor, examined the area, said he didn't notice anything serious, and prescribed an anal cream consisting of these:  cytotek, nifedipine, bethanechol, allantion powder and lidocaine.  Two questions:  he said within two weeks it should be completely healed...after about 4 days of taking it, have begun to have the constant pain again the last two days...is this normal during the healing process?  Also, the sticker says external only, should I attempt to apply any of the ointment within the opening or keep it on the outside completely?
