Hi Doc,

I have a question regarding genital warts. I am a 24 year old heterosexual male that had developed anal warts.  I was recommended to a very highly experienced colorectal surgeon, Dr. Michael Pezim (www.pezimclinic.com). When I went in for the very first visist he said that we don’t treat them here, just watch out for them and see if they resolve on their own. He also told me I had some inside me, about 2cm in.

Since that visit they had all disappeared on their own. I was happy and moved on. A while back I was curious and happened to look down there and noticed 2 pink bumps inside the anus with a white tip on them stemming from the rectal lining and got scared thinking that, those might be the internal warts he was talking about during my first visit. I made a follow up appointment with him, and saw him today.

He looked inside again, the same way he did as last time, including some sort of instrument (I think an anoscope, not sure though) and said that anal warts were not my problem, he didn’t see any, but he told me I do have a fissure with a skin tag (sentinel pile he called it) and to cut some foods out of my diet.

I asked him twice, about the warts and he said no, and I could see frustration in his face. After he was about to walk out of the door, I asked him, well how come I noticed 2 pink bumps with a white tip, he says everyone’s bum is different, stop looking and move on with life and then he walked out.

My question to you is, (since I didn’t ask him at the time cause I think he got annoyed with me)

1.Is it likely that he missed those 2 bumps that I can easily see myself by just spreading my cheeks?

2.Any ideas as to why he didn't tell me what it was, or is it so common for him to see stuff like this, that he didn't care to tell me?

3.Should I bother getting them looked at again, and if so by what type of doctor?

Thanks so much Doctor, I have been really depressed and confused over this.

To answer your questions:
1) I can't speak for Dr. Pezim.  It may be possible that the bumps are anal warts.  Sending samples out for culture as well as biopsy can be considered if there is doubt.

2) As stated before, I cannot speculate on Dr. Pezim's thoughts or actions.

3) If there is still conern, I would consider another opinion.  A referral to a dermatologist can be considered for possible culture or biopsy.

Followup with your personal physician is essential.
