Endoscopic pyloric injection of botulinum toxin A for the treatment of gastroparesis

I sure would like to know how to go about getting this done.  I live in a suburb of Phoenix, AZ (Mesa AZ) Besides Mayo?  and If anyone has had this done how did it work for you?

I've been diabetic almost 40 years have had gastroparesis for 20 years.  Raglan doesn't work and makes me jumpy.  All the other medicines that did work are now off the market.    I take Protonics, doesn't help much and Zofran again with little success.  I have changed my diet to soft food, low fat, no red meat etc., I survive mostly on a liquid diet.  I still suffer often and sometimes for weeks this last episode has lasted 3 weeks.

I do not want to go the pacer route, I already have one for my heart (I'm stage 4 CHF as well).

Anyway any help would be greatly appreciated!
