Esophageal Perforation

Hi, I have a question about esophageal negligence. How does multiple punctures impact the degree of negligence and does the size of the punctures matter? What if there are many microscopic punctures? Would preconditions of the esophagus, namely strictures, play a role in the seriousness of negligence?
