Gastris, H Pylori, Tubular Adenoma, Intestinal Metaplasia - HELP

I recently had an endoscopy after being put on Prevacid 30mg daily for one month.    During the EGD my esophagus stomach and Duodenum looked fine.  However an are of "inflamation (inflammation)'was seen on my stomach. It turns out esophagus is fine, stomach- has gastris and severe intestinal metaplasia, H plyori not seen, but blood test after confirmed H Pylori antibiodies.Also, the area of inflammation was diagnosed by pathology as benign tubular adenoma.   I was treated for the H Pylori with Prevacid 30mp BID, Amoxicillin 1 Gm BD, and Biaxin.  Now I am soon going to have another EGD done to remove the tubular adenoma.

Here are my questions:
1> Why did the H Pylori not show up on the stomach biopsy but antibiodies did in the blood test?  Does being on Prevacid for 30 days prior to the EGD biopsy effect this?

2. If appear Tubular ademona in the stomach is rare, why would it grow there?  Does it make me more susceptible to getting gastric cancer?

3. "Extensive intestinal metaplasia" (in the stomach) is making me nervous. What exactly does this mean? How likely is this to turn into gastric cancer?  If it is likely to turn into gastric cancer how long of a process is this?

Also, if the H Pylori is now gone, is there anything I can do to reverse some of the metaplasia?

4.  Should a have a breath test done, to make sure the H Pylori is gone? (I know the antibiodies will stay elevated for years, so that would not be a good indicator).
