Hemorrhoids or anal fissure

I am a healthy female 48 yrs old.  I have had problems with anal itching, what I considered hemorrhoids before.  I always used Prep. H and Tucks and they cleared up.  For the past several months though, I've had the same symptoms, but now with a small amount of bright red blood on the toilet paper, sometimes immediately after a bowel movement, other times I can tell something has leaked out and it's been again a small amt. of blood.  I have been to my dr. and they have me on anamantle for hemorrhoids.  They say if they're not cleared up in a week, they will refer me to a GI.  BTW, they didn't see anything or feel anything.  THey did notice a skin tag though. I really feel like the blood is coming from something on the outside.  They did do some sort of test on the glove used in the exam that should reveal blood and there was none.  Couldn't this just be that I'm irritated on the outside?  The reason I thought it was hemorrhoids was because it felt like I had sticks in there and big movements hurt.  Any suggestions or advice?  Needless to say, I'm a little worried that something else is going on, but I don't have any bouts of diahrrea or any abdominal cramping.  The blood is only maybe once/twice a week and is in very small amts.
