Hiatal hernia causing chest pain, fastbreathing, HBP

Age 81, female. weight 180, was 200. Activity like getting down onto toilet, stepping onto high van step, bending over to lift a box or bag full of grocery cans, sometimes just getting out of bed, is causing feeling in front of throat first, then fast breathing and pressure discomfort down center of chest.  If I wait about 9 minutes, it lets up.  If I take a nitro, it goes away in about 5 minutes.  It doesn't happen again until I do similar things. BP at home usally from 137 to 145 over 76, pulse 77. I had a heart attack in 2005, center section, refused stents & Warfarin, took some meds.  Found out then I had a rather large hiatal hernia.  Had care of my husband, so no operation. ECG in 2006 was fine.  No angina.  Now have edema in ankles. Went into emergency with this chestpain from climbing into van, fast breathing on Aug. 8.  No pain when got there., ECG normal sinus rhythm, says test indicates weakness, BP 203/110. BP down with drugs. The most unfavorable results in lab were Neutrophils% 86.1, Lymphocytes% 10.1 Lymphocytes#  0.7. The Dr. there just considered hypertension and edema and given drug prescription for BP and edema.  I know it is more than that. Did not take med (Enalapril/Hetz 5/12.5)). Who should I see FIRST--a gastroenterologist, kinesthesiologist, cardiologist for this problem? Could the Vagus nerve be affected from the hiatal hernia and the activity pressure? I don't want to go to emergency here again,  Must find better help. Small town hospital. I take coenzyme Q10 and Hawthorn and naturopathic edema control. Avoid pharmaceuticals as much as possible. Husband recently died from bleeding=side effects from SMZ/TMP DS oral antibiotic.

My Husband has GERD and he gets the same thing you described, and he avoids bending over as it does make it worse, he chews Tums and swears they make it better, You have had a bad time recently on loosing your Husband I hope you have some kind Family members to visit you, sounds like you have been through a lot reading your Post and my guess is you are very strong, maybe your question would get a better answer if put into the Doctor Forum.
